Step 3: Submit Documents

Your application is not complete until all necessary documents are received. Please send copies and do not send originals.

If you are completing the online application, you will be given directions about how and when to submit these documents. If you are completing the paper application, please follow the directions on the application form.

  1. Legal proof of age

We need legal proof of age for the student (e.g. birth certificate, baptismal certificate, passport, or immigration certificate. If none of these documents are obtainable, the parent may complete the Affidavit of Birth Date).

  2. Proof of Residency

Since we need to have proof that you live in one of our nine counties (Amador, Contra Costa, El Dorado, Placer, Sacramento, San Joaquin, Solano, Sutter, or Yolo), please submit a copy of a cable, lease or utility (electric, telephone or gas) bill showing the name of the applicant's parent or guardian. If your cable or utilities are in someone else's name, you may submit both a copy of their bill and a signed letter from them stating that you live at their address.

Students applying for grades 7 through 12 must submit copies of academic records:

    • For middle school (grades 7 or 8), submit a copy of your most recent report card.
    • For High School:
      • If you are applying for high school grade 9, either submit a copy of your most recent report card (if this will be your first ninth grade enrollment) or a copy of your high school transcript (if you have alread attended some ninth grade).
      • For grades 10, 11, or 12, submit a copy of your high school transcript.

Step 4: Attend Orientation

After your application has been processed, you will receive a call to schedule your orientation meeting.

Most students come to our center to meet our staff and complete their enrollment. If you need special accommodations to complete your enrollment let us know. -- We will help you.

The orientation meeting is for both the student and the student’s parent(s) or legal guardian(s). During this orientation you will learn more about our program and meet with a counselor to schedule classes. Depending on your location, your orientation may be at one of Visions' centers, or a staff member may come to you. All students except those in Kindergarten and first grade will also take a short reading test.
