Documents for a Complete Application
Your application is not complete until all necessary documents are received. You will be given directions about how and when to submit these documents. Please send copies and do not send originals.

1Legal proof of age:
We need legal proof of age for the student (e.g., birth certificate, baptismal certificate, or immigration certificate.) A passport with an Affidavit of Birth Date is acceptable for proof of age. If none of these documents are obtainable, the parent may complete the Affidavit of Birth Date.

2Proof of Residency:
Since we need to have proof that you live in one of our nine counties, please submit a copy of a cable/internet, lease/mortgage or utility (electric, landline telephone or gas) bill showing the name of the applicant’s parent or guardian. If your cable or utilities are in someone else’s name, you may submit both a copy of their bill and a signed letter from them stating that the parent and student live at their address.

3Immunization and Health Records
All students must submit a copy of their immunization record. All students entering 7th through 12th grades will need proof of an adolescent whooping cough booster shot (Tdap) to apply.

Enrollment Appointment
After your application has been processed, we will attempt to match your student with an available credentialed teacher in your area. Once a teacher has been assigned we will contact you to schedule your enrollment appointment.
An Enrollment Specialist will call you to complete your student’s enrollment. During your conversation you will learn more about our program, how to contact your teacher and how to order curriculum and supplies. We will also try to answer any questions that you may have.

Health Notifications
Students entering Kindergarten or 1st grade must provide a Health Examination For that has been completed by their doctor no earlier than 18 months prior to the December the student will attend 1st grade.
Students entering their first year of public school in either Kindergarten or 1st grade must have a licensed dentist or other licensed or registered dental health professional complete and submit an Oral Health Assessment/Waiver Form prior to May 31st of the student’s first year.
All students entering 7th through 12th grades will need proof of an adolescent whooping cough booster shot (Tdap) to apply.
